Ending homelessness in San Francisco
To support a more effective and accountable system, we have identified five key areas that make up a Continuum of Care (CoC) model. This framework is designed to be measurable, community-driven, and grounded in cross-sector collaboration. These five areas offer a structured approach to reducing—and ultimately ending—homelessness, while building long-term pathways to stability for unhoused individuals.

Across the San Francisco Bay Area, many nonprofits, service providers, and grassroots initiatives are actively engaged in one or more of these focus areas. Below is a selection of local organizations, themes, and topic areas that illustrate both the ongoing challenges and some of the innovative solutions being explored to end homelessness in our region.
